We’re on the hunt for a Care Coordinator to join us in our DCA Office in Bridgwater. This service is a welcoming and friendly environment with a dynamic team that supports a fantastic group of people!

As a Care Coordinator, you'll play a pivotal role in ensuring the people we support are kept safe by ensuring our care delivery colleagues are deployed effectively and that all administration is undertaken in line with relevant regulations and legislation. You’ll be responsible for managing and coordinating the care delivery team, ensuring that the relevant documentation is in place for any new people we support, discharges, and transfers. You’ll conduct live call monitoring using bespoke software and be the first point of contact for day-to-day operational requirements of our care colleagues. You will also work alongside the management team to coordinate with families, healthcare professionals, and external agencies, handle enquiries, schedule appointments and meetings, and handle all information with confidentiality. You will also take part in the on-call rota.

This is why at this service our Care Coordinators should have experience working in Health & Social Care, experience in the use of Microsoft Office, be computer literate, have prior experience in an administration role, and have experience providing excellent customer service.
