The Social Prescribing Service aims to put older people in personal control of their health & wellbeing and provide them with integrated care and support in their local community, focusing on prevention of ill health and the promotion of wellbeing. The Service aims to enable older people to have improved health outcomes and independence whilst reducing the burden on NHS services in Sunderland through the reduction in need for GP visits, out of hours contacts, and emergency admissions/readmissions.
Responsibilities:
- Deliver the Social Prescribing Service Programme across the City of Sunderland
Qualifications:
- Good standard of education at NVQ Level 3 preferably in Health & Social Care
Additional Requirements:
- Must drive and have access to own vehicle for work
- Post subject to DBS Enhanced Disclosure
