Position Overview: Provide one-to-one support for people with mild to moderate dementia and their carers through tailored personal plans, enabling them to live fully and actively in the community.
Key Responsibilities
- Develop and implement individual personal plans for service users.
- Deliver one-to-one support and care to dementia patients and their carers.
- Communicate effectively, both verbally and in writing, with service users, carers, and team members.
- Use information technology skills to record and manage user information.
- Work collaboratively within the Essence Service team.
Required Qualifications and Skills
- Experience and knowledge of working with individuals with dementia.
- Excellent verbal and written communication abilities.
- Strong IT skills.
- Full clean driving licence and access to a private vehicle.
- Will be subject to a DBS Enhanced Disclosure check.
