About the Service
The CIT Social Prescribing Service is designed to put older people in personal control of their health & wellbeing, providing integrated care and support in the local community, focusing on prevention and promotion of wellbeing. The service aims to enable older people to have improved health outcomes and independence, whilst reducing the burden on NHS services in Sunderland by reducing GP visits, out-of-hours contacts, and emergency admissions/readmissions.
Key Responsibilities
The successful applicant will be responsible for delivering the CIT Social Prescribing Service Programme across the City of Sunderland.
Qualifications
NVQ Level 3 in Health & Social Care (preferred).
Requirements
- Must have a valid driving licence and access to a vehicle
- Must be subject to DBS Enhanced Disclosure
