About Waythrough: Waythrough was formed in 2024 following the merger of Humankind and Richmond Fellowship. Together, we’ve created one of the largest mental health and social support charities in England. Every year, we support around 125,000 people through nearly 200 services – and it’s all made possible by our 3,500 brilliant staff and volunteers.

The Role: We are looking to recruit a highly motivated Health Navigator to work alongside South Tyneside Adult Recovery Service (STARS) and local partners to improve health outcomes for individuals in South Tyneside, particularly those who sit within inclusion health groups and may be socially excluded and/or experience multiple overlapping risk factors for poor health.

The most important things you’ll bring to this role are your positive attitude, enthusiasm, integrity and ability to use initiative as well as your belief in our ambition to reduce barriers and improve experiences ensuring individuals have better access to health and care services.

You will provide case management support to individuals to have better access to healthcare, improving experiences and reducing barriers. You will be required to facilitate service user access to treatment services, community assets and resources to increase health capital, ensuring that service users are informed about their options and opportunities.

Key Accountabilities:

  • Engage individuals across South Tyneside providing a range of health advice, guidance and interventions to enable engagement with health-related support within the service and the community.
  • Manage an agreed service user caseload, providing assessment, care planning, and ongoing interventions where required to improve health outcomes.
  • Facilitate service user access to treatment services, community assets and resources to increase health capital, ensuring that service users are informed about their options and opportunities.
  • Work in collaboration with South Tyneside Adult Recovery Service to support access to structured treatment for those appropriate, offering support to engage where needed.
  • Work closely and collaboratively within the multi-disciplinary team across internal and external partnerships, providing advice and information about the service offer as well as attendance at MDT and reviews.

Essential Criteria:

  • NVQ Level 3 in Health & Social Care (or above or equivalent) or willingness to complete a Level 3 qualification with 2 years or more of relevant experience.
  • Knowledge and understanding of the issues facing people who have multiple disadvantages.
  • Experience of keeping effective records including documenting relevant information onto a management information system.

Please note due to the nature of this role, a full UK driving license and access to a vehicle is essential.
