We are seeking a compassionate and proactive individual to join our team in Leeds as an Outreach and Hub Worker, dedicated to supporting people who are rough sleeping or at risk of homelessness. You will work across nights and weekends, delivering street outreach and night shelter support to help individuals make positive changes and move towards stable accommodation. This is a hands-on role that involves engaging with people at all times of day and night, building trusting relationships, and empowering them to express their strengths and aspirations while taking steps towards a more settled future.

Hours: 37.5 hours per week, Full Time
Salary: £27,861.26 – £32,002.35 (based on full time hours, pro rata for part time)
Contract Type: Fixed Term for 12 months
Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.
About the Role
- Going on outreach shifts; these include early mornings, nights, and weekends. The service covers 7 days.
- Engaging with rough sleepers and people involved in street based anti-social behaviour.
- Working in the Winter night shelter/winter accommodation overnight in shifts over a 7 day period.
- Working with partner agencies to support service users to access accommodation, physical and mental health services, substance use treatment services, benefits etc.
- Accurately recording interactions with the people we work with, other professionals, and related support plans.
About You
- Experience of engaging and providing meaningful support to individuals with complex support needs.
- Experience of communicating effectively with people requiring support.
- Experience of working in a hostel/supported housing environment.
- Experience of recording relevant sensitive information in a timely manner.
- Knowledge of at least one of the following issues around homelessness; the benefits system, substance use, mental or physical health, the criminal justice system, housing (Social and/or Private Rented).
- Some experience of supporting people who may be affected by homelessness, poor mental health and substance use.
- Knowledge of Safeguarding Procedures and their application.
- Experience of working with people in crisis.
