Healthcare Facilitators provide services that respond to changing demands and meet the needs of older people, especially those assessed as living with frailty. They support the Operations Manager and Service Coordinators to deliver person‑centred, strength‑based interventions and promote the interests of older people in Leeds.

Key Responsibilities

  • Maintain appropriate records and assist the Service Coordinators in collecting and reporting on activities.
  • Collaborate with statutory and third‑sector partners to ensure services are delivered effectively within the health and social care system.
  • Deliver person‑centred/strength‑based interventions for older people with frailty and emerging complex health and social needs, focusing on reducing the risk of avoidable hospital admission and re‑admission.
  • Participate in activities that promote the interests of older people in Leeds.

  • Problem‑solving skills and knowledge of the issues affecting older people and their families/carers.

Requirements

  • Full driving licence and access to a vehicle for business use.
  • Full training will be provided.

Hours

  • Part‑time, 28 hours per week (average) – changeable rota based on service needs. Service hours may be between 08:30 and 19:30, 7 days per week, and may alter based on service needs and/or funding. Overtime/TOIL may be available with prior agreement from the Operations Manager & Service Coordinators.
