A South London ICB is seeking an experienced Learning Disability & Autism Manager to support the delivery of its Learning Disability and Autism Programme, helping to reduce inappropriate hospital admissions and improve outcomes for children, young people and adults with Learning Disabilities and/or Autism. This is an initial 6-month contract, offering 37 hours per week with flexible hybrid working. Attendance on site is only required when needed.

The Role
You will play a key role in supporting individuals with Learning Disabilities and/or Autism to remain in their local communities and avoid unnecessary hospital admissions. Working closely with NHS providers, Local Authorities, Social Care, Education and wider MDTs, you will manage complex cases and support effective discharge planning.
Key responsibilities include:
- Chairing and coordinating CTRs and CETRs
- Managing and overseeing Dynamic Support Registers
- Supporting admission avoidance and discharge pathways
- Working with community MDTs, Social Care and specialist LD services
- Identifying barriers to discharge and escalating risks
- Supporting individuals with complex Learning Disability and Autism needs
- Providing specialist advice and guidance to professionals and stakeholders
- Producing reports and updates for commissioners and senior leaders
Requirements
- Professional background as an RNLD, RMN, Social Worker or similar health/social care professional
- Significant experience working with Learning Disability and Autism services
- Experience within community, inpatient, assessment and treatment or forensic settings
- Strong knowledge of CTRs, CETRs and the Learning Disability & Autism agenda
- Experience working across NHS and Local Authority services
- Understanding of the Mental Health Act and Section 117
- Ability to manage complex cases and work autonomously
- Excellent stakeholder management and communication skills
