We are currently recruiting for a Locum Occupational Therapist to join a Discharge to Assess (D2A) service in Cheshire. This is a rewarding opportunity to support patients transitioning from hospital to home, helping to facilitate safe and timely discharges while promoting independence.
Key Responsibilities:
- Complete functional assessments in patients' homes following hospital discharge
- Support safe discharge pathways and reduce hospital length of stay
- Recommend and arrange equipment and minor adaptations
- Work closely with MDT colleagues including Physios, Social Workers, and community teams
- Develop patient-centred care plans to maximise independence
Requirements:
- HCPC registered Occupational Therapist
- Experience within community, discharge to assess, or intermediate care settings
- Strong understanding of discharge planning and community rehabilitation
- Ability to work independently and manage a varied caseload
- Full UK driving licence and access to a vehicle (preferred)
