Vitalis are recruiting for an experienced Social Worker to work in a South Complex Care team in Westminster. Westminster City Council is seeking a skilled and motivated Social Worker to join our South Complex Care Team. This role is based in Victoria with a hybrid working arrangement, requiring regular office attendance with the flexibility to work from home as agreed with the manager.
You will be responsible for:
- Conducting Care Act assessments
- Developing care plans
- Performing risk assessments
- Reviews and placements
- Continuing Healthcare (CHC) assessments
- Mental Capacity Assessments (MCA)
- Managing referrals
Experience using MOSAIC or FrameworkI (FWI) systems is advantageous, and the role requires strong report writing skills and proficiency in MS Word, Outlook, Teams, and other MS Office applications. Additionally, you will lead safeguarding enquiries and work closely with the Safeguarding Operational Team.
