Vitalis are recruiting for an experienced Social Worker to work in a Physical Disability and Older People’s team in Reading. The PDOP Team works closely with other teams involved with the delivery of Adult Social Care. The PDOP team forms part of the Living Well Service of ASC where the Mental Health and Learning Disability and Autism (LDA) team are also members. A Head of Service oversees all work and responsibilities of the Living Well Service.

PDOP is a multi-disciplinary team comprised of Occupational Therapists, Social Workers and Social Care Co-ordinators. We provide the next step in assessment and support planning to people who are 18 years and over and have a long-term physical disability or are over 65 years old. The Social Worker will need to be within commuting distance of Reading due to the need for client/site visits and to attend the office for meetings.
