Wrexham County Borough Council is seeking an experienced Agency Social Worker to join the Team for Older People based at Crown Buildings, Wrexham.
This is a varied hybrid role combining office-based working, visits in the community and hospital settings, and home-based working.
You will manage a caseload of older adults, working collaboratively with citizens, their families, carers and partner agencies to promote independence, maximise wellbeing and achieve positive outcomes.
Participation in the team's duty rota is required, supporting the management of new referrals, urgent enquiries and priority work.
The successful candidate will undertake strengths-based assessments, develop and review care and support plans, complete Mental Capacity Assessments and Best Interest decisions, and respond to safeguarding concerns in line with legislative and professional requirements.
The role requires effective risk assessment and decision-making skills, high-quality recording and report writing, and the ability to build positive relationships with citizens, carers, family members and professionals across health, social care and voluntary sector services.
