Qualified Social Worker, Mental Health Team Pay rate £32 per hourVitalis are recruiting for an experienced Social Worker to work in a Community Mental Health team in Oldham.
- undertaking Care Act assessments,
- arranging care packages if needed & monitor its effectiveness.Â
- Monitoring MH and supporting people to remain well.Â
- As part of an MDT you will be working with health colleagues to manage risk.
- Record notes on Paris system.
