Qualified Social Worker, Early Adulthood TeamPay rate £30 per hourContract role Vitalis are recruiting for a Qualified Social Worker to work in an Early Adulthood team in Stockport.This is an exciting time to join The Early Adulthood Team as we grow to best support the residents of Stockport prepare for adulthood. We are looking for a Social Worker who can support young people with Special Educational Needs and Disabilities (SEND) to develop their goals and aspirations. Support them to work towards employment, independent living, good health and having an active role in their community.Your Role
  • To work with young people 14+ to support them to develop their skills and independence in preparation for adulthood.
  • To assess young people and adults under The Care Act.
  • To provide an assessment, support planning, monitoring and reviewing (face to face and via telephone) and work with people and / or carers to ensure that:
    • Vulnerable people are safeguarded and supported in managing risk.
  • To support people to identify their outcomes.
  • To work in a strengths-based way.
  • To carry out annual reviews.
  • To take part in a duty rota.
  • To work as part of a multi-disciplinary team
