Vitalis are recruiting for an experienced Social Worker to work in a Hospital Discharge Neighbourhood Team in Leeds City Council. We are currently recruiting a Team Manager for the hospital discharge neighbourhood teams (HDNT) to support the delivery of statutory duties and timely discharges.
As a Team Manager you will lead, motivate and direct a team of professional social workers and deliver a high standard of social work whilst ensuring that the statutory duties and responsibilities of the Local Authority are being met.
