Contract role
Vitalis are recruiting for an experienced Social Worker to work in a community team in Portsmouth Council. The social worker will have an allocated caseload predominately older persons and physical disabilities. They will be required to undertake care act assessments, Mental Capacity assessments, support plans and reviews. A laptop and mobile phone will be given and they will also have access to the AI tool Magic Notes. Flexible working can be considered.
