Family Lead Practitioners, Reading Borough Council Pay rate to £25 per hour Contract role Vitalis are recruiting for experienced Family Lead Practitioners to work in a Families First Team in Reading Council. We’re looking for a dedicated and compassionate Family help lead practitioners to join our Families first service, supporting children, young people and families being supported on child in need plans to achieve positive and lasting outcomes. To work within a social work pod as part of a Families first teams providing effective family support interventions to families accessing a S.17 child in need support plan. To work with and support a caseload of families to achieve positive outcomes, progressing interventions identified within the child in need assessment, enabling Family Led Decision Making and team around the family meetings, co-producing outcomes-focused plans with the young person/family to conduct reviews to ensure positive and sustained progress is being made. To organise and chair child in need review meetings and report back to line manager within supervision. Act as the main point of contact and advocate for families needing support, coordinating services to address needs early and prevent escalation.
