Vitalis are recruiting for a Social Worker to work in a Hospital Discharge Team in Salford Council.
Advanced Practitioner required for a Hospital Discharge Service covering Salford Royal Hospital to support staff with day-to-day tasks and any arising complexity.
Office based on the hospital grounds working Monday to Friday 08:00 to 16:00. - Interviews will also be face to face
12-month contract in support of maternity leave cover.
