Vitalis are recruiting for a Qualified Social Worker to work as a Team Manager in a Transitions Team in Kirklees Council.
We are seeking an experienced Agency Team Manager to lead our Specialist Adult Pathway Team (SAPT), supporting young people with disabilities and complex needs as they transition from Children's Services into Adult Social Care. SAPT works with young people who are likely to have eligible Care Act needs in adulthood, ensuring timely, coordinated and person-centred transition planning.
The Role
You will provide operational leadership to a team of Social Workers and Community Assessment and Support Officers, overseeing assessment, care planning, safeguarding, performance management and service delivery. The role requires close partnership working with Children's Services, Education, Health, Commissioning and provider organisations to ensure effective transition planning and positive outcomes for young people and their families.
Key Responsibilities
- Day-to-day management of the SAPT team.
- Oversight of transition assessments and Care Act duties.
- Management of performance, demand and workflow.
- Chairing complex case discussions and decision-making forums.
- Providing supervision, support and professional challenge to staff.
- Driving improvements in quality, timeliness and outcomes.
- Working collaboratively with internal and external partners to support seamless transitions into adulthood.
