We have an excellent opportunity for a Palliative Care Clinical Nurse Specialist (CNS) Band 7 to join our team on a 12 month fixed term contract. If you are passionate about palliative care but are not sure if the specialty is the right one for you, this is the chance to work with the team and understand what it takes to be a Palliative Care CNS.

As a Specialist Palliative Care CNS you will provide expert Palliative Care nursing advice, to support and facilitate high quality, individualised care to patients and relatives/carers, in collaboration with the multi-professional team, working in line with local and national targets. The successful candidate will be trained to manage their own caseload of patients, with appropriate input from the senior team, working as part of the multi-disciplinary Specialist Palliative Care team. They will maintain effective communication with patients, families/carers and healthcare professionals to ensure seamless service delivery.

To facilitate the provision of quality nursing care and to contribute to decisions regarding clinical care and priorities you will be supported to undertake comprehensive Palliative Care assessments; identifying/diagnosing patients' physical, social, psychological, cultural and spiritual needs.

Responsibilities

  • As a key member of a multi-professional team, ensure appropriate referral and discussion with other members of the team to optimise patient care.
  • Ensure patients are assessed and have care plans that are implemented and evaluated in accordance with Trust's evidence-based guidelines and protocols.
  • Maintain open and transparent communication channels that support the multidisciplinary team to do the right thing for people we care for and to speak up when things do not go as planned.
  • Act as a clinical expert for palliative and end of life care.
  • Ensure staffing is in line with Trust policy.
  • Manage resources efficiently and effectively.
  • Champion a culture of learning and quality improvement within the team.
  • Be an ambassador for palliative and end of life care across the Trust.

Essential qualifications

  • Appropriate NMC registration (Adult Nursing)
  • Degree level study or willingness to work towards Palliative care qualification
  • Mentorship/ Teaching qualification
  • Advanced Communication course or similar

Desirable qualifications

  • Master's degree or working towards in relevant subject
  • Advanced physical assessment qualification
  • Non-medical prescriber, or ready and willing to train as such

Essential experience

  • Extensive experience within Palliative/ end of life care/long term conditions
  • Experience of managing a clinical caseload
  • Experience of delivering educational sessions and developing training programmes
  • Ability to deal with difficult emotional situations

Desirable experience

  • Recruitment and selection skills
  • Evidence of leadership skills
  • Experience with managing change in the workplace
  • Assertiveness skills

Essential skills

  • In depth knowledge of speciality and use of evidence-based practice
  • Ability to work autonomously and as part of a team
