A new and exciting opportunity has arisen to join the kidney transplant team. Kidney transplantation is the treatment of choice for patients with chronic kidney disease. As a kidney recipient coordinator CNS, you will be part of a dynamic and friendly multidisciplinary team facilitating the promotion, development and enhancement of the kidney transplant pathway.

As part of your role, you will be expected to:

  • Develop, implement and evaluate a specialist service in the assessment of patients with kidney failure considered for kidney transplantation.
  • Act as a clinical nurse specialist and demonstrate a high level of expertise within the specialist service by providing advice, education and support to health care professionals and patients.
  • Act as a resource to other professionals and work closely with other members of the transplant team and external agencies.
  • Actively undertake clinical audits and research within the specialist area.
  • Contribute to the ongoing development of clinical practice, research and standards of care within the service, including development of policies, protocols and guidelines in collaboration with multidisciplinary colleagues.
  • Ensure the safety and well-being of service users, always working in a manner that promotes dignity and human rights through the adoption of person-centred care principles.
  • Provide specialist, high quality, expert nursing care and support for patients with kidney failure and their carers in liaison with the multidisciplinary team(s) and other health agencies.
  • Act as a resource for patients and the clinical team by providing comprehensive advice, information, teaching and training both within the Trust and externally.
  • Undertake, where appropriate, nursing research with published outcomes, updating own knowledge to promote excellence in clinical practice and the setting of quality standards within the renal transplant speciality.
  • Contribute in audit processes.
  • Formulate an annual report/clinical audit of own CNS service and outcomes.
  • Be actively involved in the development of the service locally and nationally.
  • Provide sound clinical leadership and develop support mechanisms for sharing good practice within the clinical setting and Trust wide.
  • Maintain and update own clinical expertise and contribute as a role model to ensure the delivery of nursing care is at the highest possible level to meet patient care.
  • Be actively involved in the development of Trust policies and procedures, providing specialist input, leading their implementation and reinforcing their use.
