We are seeking a compassionate, reliable, and experienced Supported Living Carer to provide dedicated 1:1 presence-based support for a gentleman in his 50s living in the Worthing area. The individual has Dyslexia, Type 2 Diabetes, Learning Difficulties, and complex physical health needs. This role requires a person-centred approach, strong clinical awareness, and the ability to promote independence while ensuring safety and wellbeing.

Key Responsibilities

  • Prompting and supporting with personal care and hygiene routines.
  • Assisting and supporting with insulin administration.
  • Accompanying and supporting attendance at all medical appointments.
  • Monitoring risks and maintaining health and safety within the home and community.
  • Providing guidance and supervision to reduce risks of harm.
  • Supporting with nutrition, hydration, meal planning, and food preparation.
  • Following and maintaining structured daily routines.
  • Supporting participation in activities, community access, and social interaction.
  • Assisting with mobility needs, including wheelchair use for longer distances and managing falls risks.
  • Monitoring health and wellbeing and reporting any concerns promptly.
  • Maintaining accurate records and documentation in line with company policies.

Essential Experience and Knowledge

  • Applicants must have previous experience supporting adults with complex physical health needs and be able to demonstrate knowledge and understanding of:
  • Insulin-dependent diabetes management.
  • Diabetic complications, including neuropathy and retinopathy.
  • Safe medication administration.
  • End Stage Renal Failure (ESRF) and dialysis support requirements.
  • Recognising signs of physical deterioration and escalating concerns appropriately.
  • Anticoagulant therapy and associated bleeding risks.
  • Supporting individuals with visual impairments and sensory deficits.
  • Skin integrity management and pressure area care.
  • Monitoring, recording, and reporting changes in physical presentation or health status.
  • Nutrition and hydration requirements associated with diabetes and renal disease.
  • Accurate clinical documentation and record-keeping.
