The Care Coordinator at Vibe Care is responsible for ensuring that our clients receive the highest standard of care; working in close collaboration with the Registered Manager to ensure that all care plans are meticulously developed, implemented, and monitored. This includes assessing client needs, creating tailored care plans, and making necessary adjustments to ensure the continued well-being and satisfaction of our clients.
The home care coordinator is responsible for organizing and overseeing home care services for clients. This includes communicating with clients and their families, scheduling appointments and visits, coordinating with healthcare professionals, and ensuring quality care is provided. The ideal candidate for this role is detail-oriented, compassionate, and has excellent communication and organizational skills. Previous experience in healthcare or social services is preferred.
The Care Coordinator is responsible for ensuring that all care services adhere strictly to regulatory requirements and organisational standards. This entails maintaining accurate records, managing compliance with relevant policies, and fostering effective communication between clients, their families, and care staff.
Key Responsibilities:- Client Coordination: Schedule and manage client appointments and care visits, ensuring that all care plans are followed accurately and consistently.
- Communication with clients and professionals: Serve as the primary point of contact for clients, their families, and our care staff, addressing any concerns or queries promptly and professionally.
- Care Plans: Develop and update care plans in collaboration with the Registered Manager and other healthcare professionals, ensuring they meet the specific needs of each client.
- Compliance: Ensure all care services comply with relevant regulations and standards, maintaining accurate records and documentation.
- Quality Assurance: Monitor and evaluate the quality of care provided, identifying areas for improvement and implementing necessary changes.
- On Call Rota: This position includes on-call responsibilities as part of a rotating schedule. When on call, you must be available to respond to urgent issues outside normal working hours and meet agreed response times.
- NVQ Level 2 or 3 qualification (desirable but not essential).
- Familiarity with care planning and the regulatory requirements of the care sector.
- Experience in conducting care schedules and reviews.
- Experience using digital rostering platforms and care management systems (e.g. electronic rota systems, eMAR, or scheduling software).
- Excellent organisational and methodical skills with a keen eye for detail.
- Ability to motivate and support care staff.
- Flexibility and adaptability to changing schedules and responsibilities.
- Strong team player with excellent communication skills.
Other responsibilities: As directed by the Registered Manager or Head of Healthcare from time to time to assist the continued growth of Vibe Care.
