We serve residents ages 18 and above, many of whom live with complex mental health needs, chronic illness, or developmental disabilities. Our psychiatric nurse practitioners partner closely with each facility’s long-term care healthcare team to deliver individualized, high-quality mental health care for residents. Responsibilities:
- Performing face-to-face assessments and ongoing psychiatric visits with adult clients in long-term care facilities.
- Maintaining current patient records to clearly document problems, treatment plans, actions, goals, and changes as they occur.
- Educating and supporting caregivers and facility staff.
- Managing and adjusting psychiatric medications to promote stability and well-being.
Requirements:
- Graduate of an accredited Nurse Practitioner program.
- PMHNP license required.
- Experience in psychiatric, mental, and behavioral health services.
- Long-term care and/or geriatric experience strongly preferred.
- Outpatient and tele-psychiatry experience a plus.
- Willingness to travel within assigned service area.
