Pay Range
Base Salary: $53,000 – $66,000 (Commensurate with Experience)

If you’re a registered RN/RPN, this is your opportunity to deliver high-quality home care! The Care Team Clinical Lead (CTCL) is a crucial role in promoting and maintaining a positive overall client and family experience and ensuring safe and effective care in compliance with VHA's policies, Ontario legislation, and standard of practice. The primary role of the CTCL is to conduct in-home client visits to assess, develop, and monitor Client Care Plans; provide clinical support, education, and training to personal support workers for delegated acts as well as special functions. Follows up with PSWs to ensure their skills and competencies are up to date and monitor clinical documentation; liaises with Ontario Health at Home. The CTCL takes an active role in clinical focused committees and quality improvement projects.

Key Areas of Accountability

  • CTCL is responsible for initial visit and completing a thorough assessment to establish care plan according to client needs.
  • Ensures that initial and interim home visits, as well as clients' concerns, complaints are addressed and documented.
  • Provides teaching as it relates to special functions and delegation as per College of Nurses Standards.
  • Collaborates with client, family, and other members of the care team to establish a plan of care for the client.
  • Advises and supports clients and their families at home, maintaining a style that incorporates holistic approach to care.
  • Supports after hours/on-call using clinical judgement to visit client in home when required.
  • Maintains documentation standards according to the College of Nurses of Ontario and VHA's policies.
  • Supports QI projects & Process improvement.

Required Qualifications:

  • RN/RPN Degree or Diploma from a recognized college or university with CNO registration in good standing
  • Experience in clinical skills, assessing client care needs and formulating care plans
  • Experience in the use of technology tools including electronic documentation systems such as Goldcare
  • Experience in developing clear and concise reports
  • Minimum of 2-3 years experience in community health preferred
  • Familiarity with GoldCare client management program an asset
  • Knowledge of the Occupational Health and Safety Act, relevant government legislation including Employment Standards Act and the Ontario Human Rights Code
  • Valid driver’s license and use of a car on the job is a requirement
  • Available to provide coverage for on-call shifts
