As a Care Coordinator, you will be the heartbeat of the service, ensuring smooth scheduling and outstanding support for our Care Assistants and customers across the Vale. Key Responsibilities
- Scheduling and matching Care Assistants to customers in Cardiff and the surrounding area
- Ensuring seamless rota cover and smooth care delivery across the region
- Handling new referrals and updating care plans accordingly
- Working closely with the office team to maintain accurate records and reports
- Supporting Care Assistants to ensure they have what they need to deliver high-quality care
- Participating in the on-call rota (1 weekday per week and 1 weekend in 4)
Requirements
- Minimum 1 year's experience in the care sector (Senior Care Assistant level)
- Previous Care Coordinator experience
- Level 3 Health & Social Care qualification (required)
- QCF in Health & Social Care & Social Care Wales registration (desirable but not essential)
- Full UK driving licence and access to your own vehicle
- Ability to stay organised, calm, and focused under pressure
- Strong communication and people-management skills
