  • Develop and implement personalised support plans based on Structured Clinical Management (SCM) principles.
  • Promote service user choice and control, ensuring they are active partners in their support.

  • Full driving licence and ability to travel across Gwent and inpatient settings.
  • Ability to respond quickly and positively to individual needs, demonstrating flexibility and proactive support.
  • Strong relationship‑building skills, with the ability to establish trusting therapeutic relationships.
  • High emotional intelligence, resilience, and empathy.
  • Experience working with individuals with complex mental health difficulties and challenging behaviour.
  • Ability to work independently and adapt to changing priorities.

Desirable

  • Experience advocating for service users’ needs.
  • Willingness to reflect on feedback and pursue personal development.
  • Understanding of community and third sector organisations in Gwent.
  • Welsh language skills are advantageous.

This is a full‑time, fixed‑term contract until August 2027, working on a six‑week rolling rota that includes day, evening, weekend, and night shifts. If you are committed to supporting mental health and wellbeing in the Gwent community, this role offers a meaningful career opportunity.
