We are currently looking for a bright and hardworking nurse to join our energetic Integrated Discharge team at Southend Hospital to cover maternity leave from 22/02/2027 to 21/11/2027. The successful post holder will assess patients medically optimised for discharge and make referrals for ongoing community care to facilitate safe discharge from the acute trust in a timely manner.
