Job summary

Are you an experienced and ambitious Physiotherapist looking to make a real impact in urgent and emergency care? We are seeking a highly motivated Physiotherapist to join our dynamic Urgent and Emergency Care Therapy Team at Mid and South Essex NHS Foundation Trust. This exciting role offers the opportunity to deliver advanced assessment, treatment and discharge planning across the Emergency Department, Frailty Assessment Base and Acute Medical Unit. You will play a vital role in supporting admission avoidance, improving patient flow and ensuring patients receive the right care, in the right place, at the right time. Working as an autonomous practitioner within a fast-paced multidisciplinary team, you will manage a complex caseload including patients with acute medical conditions, frailty, falls, reduced mobility and functional decline. Through clinical decision-making and collaborative working with hospital and community colleagues, you will help optimise independence, promote recovery and facilitate safe discharge. We are looking for an enthusiastic clinician with strong clinical reasoning skills, communication abilities and a passion for delivering quality patient-centred care. In addition to your clinical responsibilities, you will contribute to service development, quality improvement initiatives of the integrated frailty and acute care pathways. Main duties of the job

  • To provide advanced physiotherapy assessment, diagnosis, treatment and rehabilitation for patients presenting to the Emergency Department
  • To identify patients suitable for same-day discharge, community rehabilitation, virtual ward, reablement, intermediate care and other community-based services.
  • To work collaboratively within the multidisciplinary team to develop and implement patient-centred management plans that optimise outcomes and reduce unnecessary hospital admissions.
  • To contribute to comprehensive frailty assessment and support patients with complex physical, cognitive and social needs.
  • To participate in multidisciplinary meetings, board rounds and discharge planning discussions to ensure timely patient flow through emergency and acute services.
  • To assess and manage clinical risk, ensuring safe and effective practice in accordance with professional, Trust and national standards.
  • To contribute to the planning, delivery and evaluation of staff education and competency development programmes.
  • To lead and participate in audit and quality improvement projects
  • To act as a role model for professional practice, demonstrating the Trust values and promoting a culture of continuous improvement and patient-centred care.
  • To deputise for the Band 8a Clinical Lead/Team Lead Physiotherapist as required, providing operational and clinical leadership, supporting service delivery.

Qualifications

Essential

  • Graduate diploma or degree in Physiotherapy
  • HCPC registered as an Physiotherapist

Desirable

  • Post graduate qualification in specialism

Knowledge and Experience

Essential

  • Evidence of post graduate training/development courses relevant to speciality area
  • Experience of comprehensive assessment and treatment of patients with complex medical, neurological, musculoskeletal, respiratory, and frailty-related presentations.
  • Experience supervising, supporting, and appraising junior staff and support workers.
  • Knowledge of frailty syndromes, comprehensive geriatric assessment principles, falls management, and admission avoidance pathways.
  • Ability to work independently and prioritise a complex caseload.

Desirable

  • Has undertaken an audit or research activity that had an impact on service delivery.
  • Previous band 7 experience in ED
  • Previous NHS experience
