Mid and South Essex NHS Foundation Trust in Southend is seeking an IDT Nurse Assessor to cover maternity leave from 22/02/2027 to 21/11/2027. You will assess patients medically optimised for discharge and coordinate community care referrals to ensure safe transfer from hospital.
The post requires strong communication with patients, relatives and MDTs to support timely discharges and maintain patient flow. Experience in discharge teams and acute trusts is desirable; NMC registration is essential.
