Job summary

Broomfield Hospital at Home services are seeking a motivated and enthusiastic Healthcare Assistant to join our dynamic community nursing team. This is an exciting opportunity to be part of an innovative service delivering hospital-level care in patients' own homes, supporting recovery in a more comfortable and familiar environment. You will work across the Broomfield area, undertaking a range of duties including phlebotomy, cannulation, and simple dressings, while supporting senior nursing staff with the delivery of high-quality patient care. In addition to clinical responsibilities, the role includes supporting the smooth running of the service through office-based tasks where required, such as ordering and managing stock, maintaining supplies, and assisting with telephone queries. You will play a key role in supporting patient flow and helping patients avoid hospital admission or return home sooner, contributing to improved patient outcomes and experience. The postholder will be expected to provide occasional support to the Southend and Basildon Hospital at Home teams, including transferring stock between sites and assisting with blood tests when required.

Main duties of the job

  • Work as part of the Hospital at Home team to support the safe delivery of care to patients in their own homes
  • Provide high-quality, person-centred care under the direction and supervision of registered nurses
  • Support admission avoidance and timely discharge by contributing to effective patient care delivery
  • Undertake delegated clinical tasks within agreed competencies, including phlebotomy, cannulation, and simple wound dressings
  • Work independently within competency framework or support senior nurses during patient visits
  • Travel across Broomfield area, including between hospital sites and patients' homes
  • Support the delivery and collection of clinical consumables and equipment
  • Assist with the day-to-day running of the service, including stock management and maintaining supplies
  • Respond to telephone queries and support coordination of patient visits where required
  • Support nursing team with inputting patients onto IT systems
  • Work flexibly across community and hospital-based settings to meet service needs across MSE area
