Role Description

The Care Coordinator is a full-time, on-site role based in the London Area, United Kingdom. This role involves organizing and scheduling homecare visits, matching care workers to client needs, and maintaining up-to-date care plans and records. The Care Coordinator liaises with clients, families, and health professionals to ensure continuity of care and timely response to changing needs. Daily tasks include handling inquiries, resolving service issues, monitoring service quality and compliance, and supporting care staff with information and guidance. The role also requires regular documentation, reporting on service delivery, and contributing to the improvement of care processes and client experience.

Qualifications

  • Candidates should possess strong Care Coordination and Case Management skills to plan, monitor, and adjust care packages effectively.
  • Candidates should possess excellent Communication skills to interact clearly and empathetically with clients, families, and multidisciplinary teams.
  • Candidates should possess Home Care experience and understanding of domiciliary and complex care services across different age groups.
  • Candidates should possess solid Customer Service skills to handle queries, manage concerns, and maintain high client satisfaction.
  • Relevant qualifications in health and social care (e.g., NVQ/QCF Level 3 or above, or equivalent) are beneficial.
  • Experience in a care coordination or similar role within homecare or community healthcare settings is preferred.
  • Strong organizational, time management, and problem-solving abilities, with attention to detail in documentation and scheduling.
  • Proficiency with care management software and basic IT tools (email, spreadsheets, databases) is advantageous.
  • A commitment to safeguarding, person-centered care, and regulatory compliance, with knowledge of local health and social care standards.
