About the Role

We have several opportunities available throughout the West Sussex area, including on‑site Accommodation Services and visiting community members in their homes through our Outreach and Onwards Services. We are committed to providing personalised support that empowers individuals with complex needs to live independently and with dignity. In this role, you’ll make a real difference by helping people achieve their goals, promoting social inclusion, and managing risks with compassion and professionalism.

What You Will Be Doing

  • Provide person‑centred support to individuals with complex needs, including those with mental health diagnoses, neurodiversity, or trauma backgrounds.
  • Assist clients in developing independent living skills within accommodation settings or their own homes, promoting dignity and social inclusion.
  • Collaborate with multidisciplinary teams to implement tailored care plans, monitor progress, and manage risks effectively.
  • Maintain accurate records and ensure compliance with safeguarding and health and safety standards.
  • Engage with clients compassionately, fostering trust and encouraging positive outcomes.

Essential Skills and Qualifications

  • Adaptable, proactive, and able to respond to changing needs.
  • Full training will be provided.
