Our North & Mid Outreach team operates throughout the Burgess Hill, Haywards Heath, Crawley and East Grinstead areas. Owning or being licensed to drive is mandatory for this role.

Responsibilities

  • Provide personalised, community‑based support to individuals with complex mental health needs, promoting independence within their own homes.
  • Work closely with clients to implement and review care plans, ensuring safety and well‑being.
  • Collaborate with multidisciplinary teams to deliver trauma‑informed and recovery‑focused care.
  • Maintain accurate records and report changes in client condition or concerns promptly.
  • Participate in ongoing training to develop mental‑health support and person‑centred skills.
