What You Will Be Doing
- Facilitating the delivery of person‑centred support to individuals with complex needs, including those with mental health diagnoses, neurodiversity, or trauma backgrounds.
- Assisting clients in developing independent living skills, promoting dignity and social inclusion (this may include practical daily tasks such as cooking or going to appointments with clients).
- Providing support for clients to engage in activities in their local community.
- Collaborating with multidisciplinary teams to implement tailored care plans, monitor progress, and manage risks effectively.
- Maintaining accurate records and ensuring compliance with safeguarding and health and safety standards.
- Engaging with clients compassionately, fostering trust and encouraging positive outcomes.
