The predicted salary is between 800 - 1400 £ per month.

This is an exciting opportunity to join our well established Community Services in a brand new role. We are looking for 2 dynamic and motivated individuals to join us by working in an integrated way across multiple community services. We provide healthcare and support for patients in their own homes to offer a safe and effective alternative to hospital admission, and teams that are proactively keeping people well.

Key Responsibilities

  • You will see patients in their own homes, using your own car for transportation.
  • You will help coordinate the patients' care and be an integral part of their care provision.
  • For the right candidate, a training role commencing on B3 can be considered to complete Level 5 Assistant Practitioner Diploma and progress to band 4 on successful completion of this program.
  • Working alongside GPs, ACPs, Nurses and Therapists to support the assessment of patients in their own homes, ensuring they can access the healthcare they need.
  • This will include taking bloods and basic observations to support their medical assessments.
  • You may be visiting patients on your own and will then need to feedback your assessments to the wider team, or you may be visiting with others to support their assessments.
  • Working as a community falls responder to use moving and handling equipment to return people to a place of safety after a fall.
  • Assessing for and providing equipment like walking aids and toilet frames if required.
  • Supporting the day to day running of the teams and services you work within.
  • Providing basic care and supporting personal hygiene/toileting/meal preparation for patients in crisis and requiring enhanced support to prevent them going into hospital.

Qualifications

  • To undertake this role as a Band 3, the applicant must have the required Level 3 or equivalent qualifications.
  • To undertake the role as an apprenticeship to progress from Band 3 to Band 4, the applicant must have the minimum requirement for band 3 and then also meet the entry requirements for the Assistant Practitioner level 5 apprenticeship program.
  • Progression to band 4 will be on successful completion of the apprenticeship and related study.
  • To commence on band 4, the applicant must already have level 4 or 5 NVQ or equivalent in relevant area.
