Overview
Community Staff Nurse - East (Ledbury) – Ledbury, Herefordshire. This role is within the East Locality District Nursing Team on a fixed-term contract of 14 months, with the potential to extend for a further 14 months. Note: at present we are not accepting applications from newly qualified nurses.

Responsibilities
- Provide district nursing care to housebound patients; some work may occur in a District Nurse Clinic for non-housebound patients requiring specialist district nursing skills.
- Support patients towards self-management through education, equipment provision, and lifestyle adjustments, enabling them to live as independently as possible.
- Collaborate with GP colleagues, the wider multidisciplinary team, Care Homes, and Care Agencies to deliver integrated care.
- Record all care activities electronically on EMIS, ensuring seamless service provision with GP services. Training and ongoing support will be provided to use EMIS and other required systems.
