An exciting opportunity has arisen for a Clinical Nurse Specialist to join our proactive and dynamic Cardiac Rehabilitation Team. You will lead and work as part of the team, co-ordinating and delivering evidence based clinical practice in line with current research and guidelines.
You will work closely with Cardiology Clinical Nurse Specialists, Heart Failure Cardiac Rehabilitation and Cardiology Consultants to ensure patients in Cardiac Rehabilitation receive the best care possible. This role will help develop better care pathways for Cardiac Rehabilitation patients within Cardiology.
As a Clinical Nurse Specialist in Cardiac Rehabilitation you will ensure the delivery of high quality, clinically effective care. You will work alongside other Clinical Nurse Specialists in Cardiology and Heart Failure Cardiac Rehabilitation. You will be responsible and accountable for the management of the Cardiac Rehabilitation Team.
Cardiac rehabilitation patients sometimes have complex diagnosis and you will need to meet the needs of these patients in accordance with national, local policies and initiatives. You will undertake supervision of practice, implement clinical audit and strategic development by contributing to research, teaching and supporting professional colleagues.
You will provide managerial support to the Line Manager, to include budgetary, workforce and governance responsibilities. You will be a Non-Medical prescriber, working within your scope of clinical practice as either independent or supplementary prescriber and work to develop better titration of patients in Cardiac rehabilitation.
You will help to develop better links between primary and secondary care, ensuring the Cardiac Rehabilitation Team provide evidence based advice upon discharge from the service.
