Gloucestershire County Council are seeking a Lead Commissioner for Community Reablement to work alongside operational managers to design, mobilise, pilot and scale a new Community Reablement Service. Success will be measured by the effective mobilisation and scaling of a service supporting around 4,000 people per year to live more independently.

Location: Gloucestershire County Council

Set-up: Hybrid

Duration: Initial 12 months

Hours: Full-time position (37-hours)

Key Responsibilities

  • Lead the commissioning, mobilisation and scale-up of a Community Reablement Service over an 18-month period
  • Design, deliver and evaluate a 3-month Community Reablement pilot, working with hyper-local domiciliary care providers
  • Manage the transition of existing reablement services, including procurement and the in-housing of therapy-led assessment and planning
  • Translate the approved business case into a deliverable operational model, embedding governance, workforce and performance arrangements
  • Establish and oversee benefits realisation, performance frameworks, risk management and regular reporting to senior stakeholders

Experience

  • Significant senior commissioning experience within Adult Social Care
  • Strong understanding of reablement and domiciliary care models, including prevention-focused services
  • Proven track record of translating business cases into operational service delivery at scale
  • Experience designing and implementing benefits realisation and performance reporting frameworks
  • Demonstrated ability to work collaboratively across commissioning, operations and system partners
