Rough Sleep Support Worker
We are currently supporting a Local Authority to recruit a Rough Sleep Support Worker on a temporary basis to Contribute to achieving the overarching aim of the Council’s Rough Sleeping Service to ensure rough sleeping is rare, brief and non-recurring through the provision of timely, effective and multi-disciplinary interventions.
Hourly rate: £34.75 (Umbrella)
Length of Contract: 3 months
Hours: 37 | Monday to Friday | 09:00 – 17:00
Location: Northampton
Key responsibilities:
- Deliver proactive street outreach services, engaging rough sleepers and providing rapid support, accommodation pathways and crisis intervention
- Build trusting relationships with rough sleepers, offering intensive support to access housing, healthcare, treatment services and wider support networks
- Complete risk assessments and support plans, ensuring individuals receive tailored interventions and safeguarding support
- Support individuals to develop independent living skills, sustain tenancies and move into long-term accommodation
- Work closely with Housing Options, Adult Social Care and partner agencies to deliver personalised housing plans and positive outcomes
- Coordinate and participate in multi-agency case conferences for individuals with complex needs, repeat homelessness or entrenched rough sleeping
- Work collaboratively with hospitals, prisons, landlords and supported housing providers to prevent homelessness and rough sleeping
- Act as an advocate and key point of contact for rough sleepers, ensuring access to appropriate services and support
- Contribute to rough sleeper counts, SWEP provision and wider initiatives aimed at reducing rough sleeping across the area
- Maintain accurate case records and ensure all interventions are recorded in line with policy, safeguarding requirements and service standards
