VAST is seeking an Integration Coordinator to raise the profile of the Connect to Work Programme across Stoke-on-Trent and strengthen referral pathways into the service. You will build trusted relationships with primary care teams, NHS and mental health services, local authority teams and VCSE groups to increase engagement.
You will support outreach, plan events to reach potential participant groups, and ensure all engagement is recorded for monitoring and evaluation.
