What You’ll Do:

  • Provide skilled in-home nursing visits based on the plan of care
  • Perform vital signs, medication administration, wound care, and catheter care
  • Observe and report changes in patient condition to the RN Case Manager
  • Educate patients and caregivers on medications, safety, and disease management
  • Complete accurate and timely documentation
  • Communicate effectively with the care team

What We’re Looking For:

  • Active Texas LVN license (required)
  • Home health experience preferred
  • Strong clinical and communication skills
  • Ability to follow care plans and adapt to patient needs
  • Reliable transportation & valid driver’s license
  • Kinnser/WellSky EMR experience (preferred)
