Community Rehabilitation Services provides ongoing rehabilitation to individuals following an acute episode of care. The aim of the service is to enable people to gain or regain their confidence, ability, and necessary skills to live as independently as possible upon discharge from hospital. All staff work as part of a multi-disciplinary team comprised of Occupational Therapists, Physiotherapists, Rehabilitation Support Staff and Social Workers.
What you'll do
- Support the manager in the provision of holistic person centred care within the Unit.
- Encourage and promote the core values of personhood, independence, respect, choice, dignity, privacy and fulfilment.
- Undertake the role of a key worker having responsibility for the ongoing assessment, care planning and review of the needs of service users.
- Participate in the 24 hour rosters covering daytime, evening and night time.
- Undertake day-to-day operational/management duties.
- Respond to referrals from acute hospitals and interim facilities for admission to unit.
- Assist the manager in maintaining and delivering a high quality safe and effective service.
- Assist in the development of a skilled workforce through supervision, appraisal, staff development and training of the care staff team.
- Work collaboratively with community rehabilitation staff and other agents of social and health care in the delivery of person centered care.
- Adhere to the DHSS and Trust policy on 'Use and Control of Medicines' and the community policy on 'Administration of Medicines'.
- Ensure the safekeeping of service user records and maintain accurate service user records at all times adhering to relevant legislative framework.
- Undertake risk management training and ensure adherence to health and safety policies and procedures.
- The post holder must be on the Live register of NISCC (or professional equivalent) and act at all times in accordance with their code of conduct.
About you
- Professional qualification in Nursing/Social Work/Allied Health Professions or NVQ level 3/QCF level 3 award in Care or higher and GCSE English grade C or above or an equivalent qualification.
- A minimum of three years' experience working in a professional paid capacity with older people.
- Be registered with a Professional Body i.e. NMC/NISCC or eligible to be registered with NISCC.
