Vanrath are recruiting for a Band 4 Community Care Support Worker on behalf of a large public sector company in Antrim on a temporary basis.

About your next employer

The post holder will be required to provide a Community Support Worker role with adults with a physical disability, older people and/or their Carers. This will require them to be an integral member of both the multidisciplinary team and the community social work team. They will contribute to the assessment, care planning and arrangement of service for clients where their involvement is allocated by the Senior Social Worker in the team. The Community Care Support Worker will also refer clients to access community facilities offering health, social education, and leisure and employment opportunities and follow up on referral afterwards.

What you'll do

  • Provision of social care service to clients by undertaking tasks and activities allocated by the Band 7 Senior Social Worker.
  • Source and implement the provision of domiciliary care packages to include completion of relevant documentation and information systems to support commissioning of service as allocated by the Senior Social Worker.
  • Implement or amend packages of care provided by the Trust homecare department and Independent sector providers.
  • The Community Care Support Worker will carry out Level 1 reviews and complete the Adult Social Care Outcomes Tool for level 1 case work.
  • The Community Care Support Worker will complete Self Directed Support documentation under the supervision of the Senior Social Worker.
  • Liaise with Service User/family members and/or Carers to keep them informed of any changes to services commissioned.
  • The Community Care Support Worker will complete Carers Assessment under the supervision of the Senior Social Worker.
  • The Community Care Support Worker will conduct Level 1 Direct Payment Reviews under the supervision of the Senior Social Worker.
  • Liaise with other Professional staff and agencies in the interests of the service user/family/Carer.
  • To support service users as part of a rehabilitation programme.
  • Monitor and keep under review cases and refer to Physio, Community OT and other services both within and outside the Trust according to assessed needs.
  • Support the Duty Social Worker assisting with office duty involving level 1 cases.
  • Refer service users to key social, community and voluntary networks to meet their needs and follow up on these referrals afterwards.
  • Liaise with relevant voluntary and community agencies in the provision of services for example senior citizen groups/consortium, volunteer centre and support these as required to promote Community Development and Trust Service.
  • Assist with the arrangement of admission to care homes for short breaks and day care facilities.
  • Assist the Community Occupational Therapist in completion of the transport risk assessment for service users by completing the relevant section for service users attending day care opportunities.
  • As a Trust representative, participate in the IMPACT strategic HUB meetings with GP Groups, Pharmacy and IMPACT Age Well officer.
  • The Community Care Support Worker will review community domiciliary care packages and escalate any change in need or deterioration to the senior social worker.
  • In consultation with the Senior Social Worker make adjustments to care packages as necessary by reviewing whether to reduce or increase service.
  • In consultation with the Senior Social Worker the Community Care Support Worker will carry out level 1 hospital discharge reviews in required strict timeframe, this includes the monitoring & reviewing of level 1 Rapid Response & Recovery Services and all other duties associated with discharges for hospital to a range of community service and all other duties with the transition from hospital discharge to a range of community services.
  • Use Trust wide and departmental computer technology in line with the Trust's Information Management and Technology Strategy systems such as Soscare, HRPTS, E-nisat, Brokerage, LCID, NIECR, Datixweb, CRMS, Microsoft Outlook, and any other information systems daily.
  • Assist and support with the Community Discharge Facilitator role within the team, supporting the CDF with the admission and discharge of service users to statutory and private Care Homes.

About you

  • QCF/ NVQ Level III in Care plus 1 year's paid experience caring for older/disabled people in a Community, Hospital, Day Care or Residential/Nursing setting OR QCF / NVQ Level II in Care plus 2 years' paid experience caring for older/disabled people in a Community, Hospital, Day Care or Residential/Nursing setting OR 5 GCSEs (Grade A-C) to include English Language plus 3 years' paid experience caring for older/disabled people in a Community, Hospital.
  • Hold NISCC registration or eligible for registration with NISCC on appointment.
  • Hold a current full driving licence.
