Program: EPIC Mens Residential

Pay: (pay is calculated based on certification status)

  • Uncertified Case Managers start at $17.50 - Uncertified Case Managers will obtain certification within the first 90 days and pay will be increased
  • Certified Case Manager range starts at $19.61 - Certified CM pay is calculated based on years of certified experience.

Shift Differential: additional $2/hour when clocked in for 3+ hours between 3pm-11pm

  • Evening Shift (4pm–12am) Options: Tues–Sat, Fri–Tues, Thurs–Mon, Sat–Wed
  • Extended Evening Shift (2pm–12am) Options: Sun–Wed, Wed–Sat, Sun, Mon, Thurs, Fri

Job Summary & Deliverables: The Residential Case Manager is a certified position that provides targeted case management and psychosocial rehabilitative services as indicated by the care plan to support clients in residential settings to meet their individualised goals. Certified Case Managers are responsible for utilising evidence-based practices, meeting fiscal responsibilities, and ensuring that regulatory compliance and organisational standards are met.

Essential Functions:

  • Provides targeted case management services as described by the Medicaid manual within residential programs
  • Supports clients in following individualised care plans and identifying case management goals
  • Completes documentation on services provided within Valley’s standards
  • Meets productivity expectations as assigned by supervisor
  • Distributes medications as necessary
  • Observes and collects client urine samples as necessary
  • Transports clients to appointments as necessary

Requirements

Education: High School diploma or equivalent

Experience: None - see Preferred Qualifications

Licenses/Certifications: Case Manager certification, CPR certification, Valley de-escalation certification. Driving positions require a minimum age of 21 and a current driver’s license.

Preferred Qualifications: One year social services or behavioural health experience, Bachelor’s degree in Social Services, Psychology, or related field.
