This program helps nurses transition from novice to competent clinicians, focusing on growth, critical thinking, and leadership to deliver high-quality, compassionate care. Opportunities include: Adult Home Care: Our Community Health Nurses provide skilled nursing care to patients in their homes, working to the top of their licensure. Nurses meet clients between 8 am and 4:30 pm, with a team comprising RNs, Physical Therapists, LPNs, LNAs, SLPs, Social Workers, and administrative staff, all dedicated to helping patients remain in their preferred setting. Community Hospice: Provides skilled nursing care to terminally ill patients at home, focusing on client education and palliative care to address physical and emotional needs, while supporting families. McClure Miller Respite House: Vermont\’s only Medicare-certified inpatient hospice residence, offering care for terminally ill patients.
