Job Description: We are seeking a highly motivated and experienced Band 6 Physiotherapist to join our Allied Health Professionals team. This role focuses on delivering high-quality physiotherapy care to housebound patients in the community, with a particular emphasis on fall prevention and management. Key Responsibilities: * Conduct community visits to assess and treat housebound patients, delivering tailored physiotherapy interventions. * Triage patients for the Falls Practitioner team, prioritizing urgent or high-risk cases for referral to the BHR Falls Service. * Develop and implement individualised care plans to reduce the risk of falls and improve mobility and independence. * Provide education and support to patients, families, and caregivers on fall prevention strategies. * Collaborate with multidisciplinary teams to ensure a holistic approach to patient care. * Maintain accurate documentation in line with professional and organisational standards. * Participate in service development and quality improvement initiatives within the community and falls service.
