Would you like to play a part in the Hospital at Home service in the beautiful location of Middlesbrough, Redcar and Cleveland? We have a rare opportunity for a permanent position to lead and oversee clinical pharmacy input to the Urgent Community Response (UCR) and frailty virtual ward team to deliver care to the virtual ward and Urgent Care Response teams. You will contribute to patient care both within the unit base setting and via patient home visits where appropriate.

Responsibilities:

  • Lead clinical pharmacy input to the UCR and frailty virtual ward team.
  • Deliver care to the virtual ward and Urgent Care Response teams.
  • Provide patient care within the unit base setting and via patient home visits.
  • Lead medication provision and delivery across organisational boundaries.
  • Complete medication reviews, medicines reconciliation and pharmaceutical care plans for patients admitted to the virtual ward.
  • Act as an independent prescriber, optimising medicines and promoting evidence-based, cost-effective prescribing and deprescribing.
  • Use clinical skills to investigate, analyse and implement actions for medication-related incidents within the virtual ward.
  • Provide specialist clinical pharmacy knowledge to health and social care professionals on safe prescribing, handling and administration of medicines.

Qualifications:

  • Access to a vehicle for work purposes.
  • Qualified independent prescriber.

Training & Development:

South Tees Hospitals NHS Foundation Trust offers leadership and improvement training to all new staff. The programme supports leaders in developing their understanding of leadership and management skills. The following training sessions are expected:
