Physiotherapy Assistant Practitioner, Acute Admissions Team

Join to apply for the Physiotherapy Assistant Practitioner, Acute Admissions Team role at University Hospitals Sussex NHS Foundation Trust.

This role offers a great opportunity for support worker development with very different clinical experiences every day.

We are looking for a dynamic individual to work within our front door service at St Richards Hospital. This person should be able to think on their feet, have experience with prioritising and discharge planning skills and is confident working in a positive risk taking environment and is able to assist with solving problems to get the best outcome for patients. You will do functional mobility assessments, advise and implement treatment and discharge plans for patients referred to the Front Door Service, including patients with varying medical, frailty, and orthopaedic presentations and complex social and psychological conditions.

Responsibilities

  • To provide assessment, treatment and discharge planning for a range of acute medical, orthopaedic and elderly frail patients referred to the Acute Admissions Team, Front Door therapy service, in accordance with national and local guidelines and standards.
  • To work in collaboration with the wider MDT and health professionals in both secondary and primary care.
  • To provide educational support to patients, carers, MDT and new physiotherapy staff to the emergency Floor.
  • To support the Team Leads and Band 6 and Band 5’s.
  • To participate in CPD, IST, audit, evidence-based projects and service development projects
  • To be available to work across a seven-day period/rota if/when required.
