An exciting opportunity has arisen for an experienced and dynamic person to be a pivotal part of Cancer Services. The post holder will work closely with the Lead Cancer Nurse to deliver on personalised care projects, identified in the NHS Long Term Plan. The successful candidate will have experience leading and managing teams and promoting strong visible leadership as well as confidence influencing teams across the trust. The post holder must be equipped with the skillset to lead and navigate teams through the financial climate and project managing new ways of working to improve outcomes for our patients. The successful candidate will have a good understanding of the personalised cancer agenda, specifically the benefit of stratified follow up, experience in working with charities or external parties and patient experience including NCPES.
Main duties of the job include embedding and improving personalised care interventions including:
- Holistic Needs Assessments (eHNA)
- Personalised Care Support Planning (PCSP), based on HNA
